What is a Cognitive Distortion? A cognitive distortion is when we focus on particular aspects of an experience (often 'negative' ones) and ignore or discount others—thereby distorting our view of the world. This free tool highlights the 10 most common distortions that people use, complete with descriptions and examples of how they might be used! As far as session notes go, I either just create a new Microsoft Word document or write them by hand. The only info I store for each set of notes is Client Name, Session number & Date. So, I'm not sure what else you would need for recording session notes - but if you have some ideas, or you have something specific in mind, do let me know.
